What do we need from you?

In order to make your Custom Acrylic sheet, we would like to receive:

• A digital ultrasound photo
  (you can usually easily request this from the hospital or your midwife)

• The birth information you want on it (if you add it)
  Think: name, date of birth, time, weight, height (and possibly location).
