What do we need from jij?

To be able to Make your plexiglass plate, we would like to receive:

• One digital ultrasound photo
  (this can usually be easily requested from your midwife)

• The birth details jij want on it (if jij add one)
  Think of: Name, date of birth, time, weight, length (and possibly place).
