You use this safety bead as the last bead on the loop side. One thread goes through one hole and a satin cord thread through the other hole. This prevents the final bead from slipping over the knot of the last loop. Even if someone removes that knot, the beads cannot come off the pacifier chain. And that is indeed very safe.
