What do we need from you?

To be able to make your plexiglass sheet, we would like to receive:

• One digital echo photo
  (these can usually be easily requested from your midwife)

• The birth details that you want on it (if you add one)
  Consider: name, date of birth, time, weight, length (and possibly place).
